Code of Conduct for Pet Therapy Volunteers: The code of conduct form which commits to ethical standards in all activities and for all volunteers, officers and directors. It requires a signature in order to proceed as a volunteer.
Facilities Request For Service Visits: The facility request for services forms’ purpose is a signed waiver of liability and indemnity agreement for any facility requesting pet therapy services of CAAA.
Insurance Form: The insurance form shows proof of homeowners insurance for liability coverage. Although we have never had an incident, if there was one, both CAAA’s volunteer insurance and the person’s home owners insurance would work together to determine any needed reimbursement.
Job Description- Social Pet Therapy Volunteer: The job description describes human qualifications and requirements in order to perform functions of a pet therapy volunteer.
Photography Release Form: The photography release form gives CAAA permission to take and publish photos of you and your pet for the purpose of promoting the organization.

Veterinary Annual Health Check Up: An annual veterinary health check up form which ensures up-to-date vaccinations and a negative fecal exam filled out by your veterinarian.
Visit Log: A visit log to be completed for each visit made in a calendar year in order to provide a total number of
visits towards pet therapy team milestones and awards.
